Ascendant Health and Longevity is a cash-pay functional medicine practice. Dr. Allen Ashforth is licensed to practice in the state of North Carolina, and telehealth services are provided only to individuals physically located in North Carolina at the time of the visit, unless otherwise permitted by applicable law.
We do not bill insurance. Health Savings Account (HSA) and Flexible Spending Account (FSA) funds are accepted where applicable; you are responsible for confirming eligibility with your plan administrator.
4. Communications
By submitting your email address or phone number through any form on the Site, you agree to receive email and/or SMS communications from Ascendant Health and Longevity, including educational content, program information, and marketing materials. You may opt out of receiving any or all of these communications at any time by following the unsubscribe link in any email or replying STOP to any SMS message.
5. Purchases
If you wish to purchase any product or service made available through the Site, you may be asked to supply certain information relevant to your purchase, including payment information. All payments are processed securely through third-party payment processors. We do not store full payment card details.
Ascendant Health and Longevity is a cash-pay practice. We do not bill insurance directly. HSA and FSA funds are accepted where applicable.
